I, the undersigned, authorize Sand Road Animal Hospital (SRAH) to provide medical care for my pet(s). I understand there is no guarantee for the outcome of treatments, and I take full responsibility to understand any risks involved. Sand Road Animal Hospital does not offer payment plans or financing for services. All payments are due at the time services are rendered. Sand Road Animal Hospital accepts all forms of payment including Scratchpay and CareCredit. Pets with insurance still need to pay at the time of service, and the insurance company will reimburse the claim directly to you.

Sand Road Animal Hospital only works with Vets First Choice and does not collaborate with any other 3rd party pharmacies. If your pet requires prescriptions and you prefer to obtain medications from a pharmacy other than the clinic or Vets First Choice, you will receive a paper prescription to use at the pharmacy of your choice. Sand Road Animal Hospital cannot fulfill requests from external pharmacies such as Chewy, Amazon, or OptumRX.
